Health History Questionnaire

Come in for a tour and we'll show you all of the great things about Health Place. We'll explain our individual, couple, family and corporate memberships-all with affordable monthly dues. No hype, no confusing offers, and no heavy sales pressure. Memberships can be paid monthly or yearly and discounted rates are offered for eligible family members. Membership includes full use of the facility,including swimming, racquetball and aerobic and group fitness classes. All new members are offered a complimentry fitness evaluation and program consultation; personal training is available at an additional charge for those wanting ongoing training. Other services, including our nursery and massage therapy are available for an additional fee. As a medical fitness center, we appeal to a diverse population and consider your health, right from the start. Prospective members fill out a health history questionnaire which is reviewed by a degreed exercise specialist. Depending on risk factors, a physician's clearance may be requested which is facilitated by our staff at no charge to you.
